  • What is the standard routine well visit schedule?

Each well visit is an opportunity for the parent to raise questions and concerns about their child's development, behavior, and general well-being. Well visits are very important from birth through college and provide the necessary physical and vaccination forms for school, daycare, sports, and summer camps. • Newborn – Within 2-5 days of your child being born • Two weeks • Two months – Vaccinations begin. Refer to the Routine Vaccination Schedule or discuss an alternate immunization schedule with your provider. • Four months • Six months • Nine months • 12 months • 15 months • 18 months • Two years • Once every year thereafter for a physical exam, hearing and vision screening, vaccinations as needed, and general questions regarding your child's well-being.
